Making a Complaint or Grievance
While we will always do our best to provide you with the best possible care and customer service, we understand that sometimes things happen that can make your experience with us unsatisfactory. If that happens, we want to know about it so we can improve, and ensure that we are consistently providing safe, high quality care for all.
Grievance Procedure
The Interior Community Health Center (ICHC) provides a variety of preventive and primary health care services to individuals living in the community.
Financial Eligibility: ICHC charges fees for its services. ICHC's charges are consistent with local prevailing rates for similar services and cover the center's costs of operation. A copy of ICHC's fees can be found on its home page and are posted in its registration booths. ICHC offers a sliding fee discount for its services to eligible individuals. Eligibility is based on household size and income. Except under exceptional circumstances, discounts may only be given to individuals and households living at or below 200% of federal poverty. Billing grievances greater than 60 days are only eligible for review by the Operations Officer.
How to File a Grievance: ICHC strives to provide quality health care services. If you believe that ICHC has unfairly denied you services or if you are dissatisfied with the services provided, you have the right to appeal, orally or in writing, to the center's Operations Officer. In presenting your grievance, please state the problem and, as specifically as possible, what relief is desired.

If the Operations Officer is unable to resolve your grievance to your satisfaction, you may ask in writing within 10 business days that your complaint is forwarded to the Chief Executive Officer:

If the Chief Executive Officer is unable to resolve your grievance to your satisfaction, you may ask in writing within 10 business days of receipt of the Chief Executive Officer's decision that your complaint be forwarded to the Board of Directors. The Board President may or may not bring the grievance to the full Board of Directors.

Grievance Form
For your convenience, a fillable copy of the patient grievance form is provided here. We ask, but do not require, that you first speak to a staff member or department supervisor to address your grievance or complaint. If you are unable to resolve the issue in that manner, please fill out the grievance form and present a printed copy to any staff member, who will forward it to the Operations Officer per the above procedure.
